There he meets Manabu Ito, a medical student who is recruiting volunteers for a surgical procedure known as trepanation.

Although not initially interested, Susumu agrees to participate in the experiment of having his skull drilled into in exchange for a large sum of money.

After the operation, Susumu discovers that he now has a new and grotesque worldview, being able to see a reflection of the traumas and insecurities that are hidden in people´s minds. This newfound sixth sense forces him to confront the monstrous and distorted "homunculi" that represent the true inner selves of everyone he encounters, blurring the lines between reality, perception, and madness.
